Your Insurance Coverage

All registered Footscray Rugby Club players are covered by Rugby Australia's insurance policy. This coverage is included in your annual membership fees and provides protection for injuries sustained during:

  • Official club training sessions
  • Sanctioned matches and games
  • Club-approved rugby activities

What's Covered

Non-Medicare Medical Expenses

100% up to $3,000

Covers private hospital, physiotherapy, chiropractic, dental (sound and natural teeth), and similar registered medical services

$100 excess applies (except ambulance)

Loss of Income

$300 per week for 52 weeks

Capped at 100% of your average weekly income from the 12 months prior to injury

28-day deferral period applies

Travel & Accommodation

80% up to $2,000

Fuel/airfare for trips over 100km round trip to medical treatment

Accommodation capped at $150 per night

Emergency transport expenses with no excess

No $100 excess applies to ambulance

Important Conditions

  • Medicare Exclusion: No coverage for expenses that Medicare covers (prohibited by law)
  • Time Limits: Medical expenses must be claimed within 12 months of injury
  • Doctor Referral Required: Referrals must be obtained BEFORE treatment begins
  • Private Health First: You must claim from private health insurance before submitting to SLE
  • Medical Certificates: Required every 4 weeks for loss of income claims (no backdated certificates)

How to Make an Insurance Claim

⚠️ Important: You must report your injury within 30 days of the incident to be eligible for coverage.

Report Your Injury Online

As soon as possible after your injury, complete the online injury report at:

This will generate a claim number - save this number as you'll need it for all correspondence.

⚠️ You cannot proceed with your claim without a claim number. This must be completed first.

Visit a Doctor for Medical Statement

Book an appointment with a doctor who will complete the RA Medical Statement to verify your injury and treatment requirements.

The doctor will assess your condition and complete the medical documentation needed to support your claim.

⚠️ This must be completed by a qualified medical doctor. Physiotherapists, chiropractors, or other non-doctor practitioners cannot complete this form.

Submit Your Claim

Submit all completed forms, medical statement, and supporting documentation. Once verified and approved, you'll gain access to the insurance policy benefits.

  • Include your claim number on all correspondence
  • Keep copies of all submitted documents
  • Wait for claim approval before accessing benefits
